Cause one: the surface never had enough fall
A driveway needs a deliberate slope towards wherever the water is supposed to go. Where it was laid flat, or falls towards the house, water has nowhere to run.
This is a design fault rather than a failure, and fixing it means regrading the affected area or intercepting the water with a channel drain in the right place.
Cause two: settlement created a hollow
If the water sits in a defined dip, especially under a wheel track, the base has settled. The fall was probably fine when it was laid.
The fix is to lift the surface locally, rebuild and compact the base, and relay — which also solves the puddle permanently rather than working around it.
- Even sheet of water: insufficient fall

- Water building at a gully or channel: blocked or failed outlet
- Water tracking towards the house: urgent — check falls immediately
Cause three: the outlet has failed
Channel drains and gullies silt up, fill with leaves, or discharge into something that has collapsed or become blocked underground.
Before assuming the driveway is at fault, it is worth lifting the grating and checking. A blocked channel is the cheapest of all these problems to fix.
When water reaches the house
Water pooling against a wall or running towards a door threshold needs dealing with promptly, because a driveway built up above the damp course is a genuine risk to the building.
The usual remedies are lowering the surface at that edge, cutting in a channel drain along the threshold, or both.
